The majority of the group will travel to Cary, MS in one day. We will leave, Saturday, March 29 and return on Saturday, April 5.
*Some people chose to travel earlier than the group. If this applies to you, please make sure Maria Bowater knows.

In the event that I am rendered unable to independently make medical decisions, I consent to any reasonable medical treatment as deemed necessary by a licensed physician. In the event treatment is required from a physician and/or hospital personnel designated by the church, I agree to hold such person free and harmless of any claims, demands, or suits for damages arising from the giving of consent. I also acknowledge that I will be ultimately responsible for the cost of any medical care should the cost of that medical care not be reimbursed by the health insurance provider.
Further, I affirm that the health insurance information provided above is accurate at this date and will, to the best of my knowledge, still be in force during the dates of this serve trip.
The Code of Conduct will be presented at the information meeting on February 25, 2024 at 6:30pm. Each participant is expected to abide to the code of conduct. If you are found in violation of the code of conduct, you will be ask to go home at your own expense.

I/We the undersigned have legal custody of the student named above, a minor, and have given our consent for him/her to attend the 2024 Spring Break Serve Trip being organized by RedArrow Ministries.

In the event that he/she is injured and requires the attention of a doctor, I/we consent to any reasonable medical treatment as deemed necessary by a licensed physician. In the event treatment is required from a physician and/or hospital personnel designated by the church, I/we agree to hold such person free and harmless of any claims, demands, or suits for damages arising from the giving of consent. I/We also acknowledge that we will be ultimately responsible for the cost of any medical care should the cost of that medical care not be reimbursed by the health insurance provider.
