Volunteer Contact Information
In case of emergency, we need to have this information on file.
Thank you for volunteering within RedArrow Home Repair Help. We have need for four types of volunteers. Please select all roles you are willing to fill.
Project Leader: The project leader will visit the site with Pastor Maria, assess the needs and determine if the project is within the capabilities of our volunteers. Project leaders will help Pastor Maria write a description of the scope of the project, estimate budget, and set schedule.
Safety Coordinator: The safety coordinator will vist the site with Pastor Maria and the project leader. At the site visit, the safety coordinator will determine if the site is a safe and secure environment for our volunteers.
General Laborer: General laborers could be skilled or unskilled workers. These volunteers will follow the instructions of the project leader to complete any repairs assigned.
Hospitality/Care Coordinator: This coordinator could schedule snacks/meals for the volunteers and create a care package for the home owner and household.
(Parent Permission BELOW- SKIP SECTION if you are a student) I understand that there are inherent risks involved in any project or activity, and I hereby release RedArrow Ministries, its pastors, employees, agents, and volunteer workers from any and all liability for any injury, loss, or damage to person or property that may occur during the course of my involvement in RedArrow Home Repair Help.
In the event that I am rendered unable to independently make medical decisions, I consent to any reasonable medical treatment as deemed necessary by a licensed physician. In the event treatment is required from a physician and/or hospital personnel designated by the church, I agree to hold such person free and harmless of any claims, demands, or suits for damages arising from the giving of consent. I also acknowledge that I will be ultimately responsible for the cost of any medical care should the cost of that medical care not be reimbursed by the health insurance provider.
Further, I affirm that the health insurance information provided above is accurate at this date and will, to the best of my knowledge, still be in force during the dates of any projects.
RedArrow Home Repair Help requires volunteers to conduct themselves in a manner that represents God, Christians and our church well. From the home owners to other volunteers, all people are created in God's image, and therefore, we expect that you will respect them.
*Please note that there is a zero tolerance policy for any and all acts of violence, inappropriate touch, harrassment and bullying.
This section needs to be filled out if you have a minor child (or adult child still in high school) volunteering with RedArrow Home Repair Help.
I/We the undersigned have legal custody of the student named above, a minor, and have given our consent for him/her to volunteer within RedArrow Home Repair Help being organized by RedArrow Ministries.
I/We understand that there are inherent risks involved in any ministry or activity, and I/we hereby release RedArrow Ministries, its pastors, employees, agents, and volunteer workers from any and all liability for any injury, loss, or damage to person or property that may occur during the course of my/our child’s involvement in RedArrow Home Repair Help.
In the event that he/she is injured and requires the attention of a doctor, I/we consent to any reasonable medical treatment as deemed necessary by a licensed physician. In the event treatment is required from a physician and/or hospital personnel designated by the church, I/we agree to hold such person free and harmless of any claims, demands, or suits for damages arising from the giving of consent. I/We also acknowledge that we will be ultimately responsible for the cost of any medical care should the cost of that medical care not be reimbursed by the health insurance provider.
